Downloadable Library Card Applications

Completed library card applications may be returned to any KCLS Library.

Follow these 4 Easy Steps to Get Your King County Library Card

Select and print the Library Card Application form from the list.
2 Fill out the form.

Bring your completed form to your local KCLS library with:

LicensePhoto identification with birth date
(one piece required for adults)
Acceptable examples:

  • Driver’s License (current or expired)
  • Passport
  • Washington ID Card
  • U.S. Military ID
  • Matricula Consular (issued by Mexican government)

AND Address verification matching qualifying residential address and given name (one piece required for adults and minors)
Acceptable examples:

  • Current WA Driver’s License
  • Washington ID Card
  • Printed Checks
  • Current Utility Bill (printout of online bill acceptable)
  • Apartment Lease
  • Voter’s Registration
  • King County Tax Bill
  • Other (Must be of a legal or permanent nature)
    If residence address differs from mailing address, the residence address determines patron eligibility.
